HISTORY

 

Separate efforts in Manitowoc and Sheboygan County to increase access to primary health and dental care for underserved residents joined to form Lakeshore Community Health Care in 2014.

 

Today residents of both counties can access primary and preventive medical, behavioral and oral health care at two convenient locations in Sheboygan and Manitowoc.

2006

Healthiest Manitowoc County started the Healthy Teeth Healthy Community program, using federal grant funding, to provide dental services to underinsured children in Manitowoc County schools.

 

United Way of Sheboygan County granted $20,000 to Healthy Sheboygan County 2020 to explore how to expand health care and dental services to nearly 30,000 uninsured and underinsured Sheboygan County residents.

2010

United Way of Manitowoc County provided a venture grant to expand Healthy Teeth Healthy Community dental services to adults. A new non-profit, Healthy Teeth Healthy Communities of Manitowoc County Inc. (HTHCMC), was created to support the expanded services.

2012

Lakeshore Community Health Care began providing dental services one day a week in Sheboygan and was awarded a Section 330 federal grant, designating LCHC as a Federally Qualified Health Center.

2013

Healthy Teeth Healthy Communities of Manitowoc County began providing medical services two days a week and changed its name to Community Clinics of Manitowoc County.

2014

In order to make the best use of federal and local grant opportunities, the Manitowoc and Sheboygan organizations merged. Lakeshore Community Health Care, Inc. became the newly combined non-profit's official name.

2017

Renovations to the Dental wing were completed at the 1721 Saemann Avenue clinic in Sheboygan, bringing all of LCHC's Sheboygan services under one roof.

 

In August, the Manitowoc facility relocated to 2719 Calumet Avenue. The move brought LCHC to a more accessible location and added space for expanded services.

2018

LCHC received a Patient Centered Medical Home designation from the

National Committee for Quality Assurance, recognizing our commitment to high-quality, integrated care and patient satisfaction.
