View Your Records and Connect with Your Providers



IMPORTANT: Your Medical Record Number (MRN) is the Account Number listed on your bill.

View and pay your bill online with a credit card, debit card or bank account. Don't forget to check "email me a receipt" if you would like one for your records. Payment is due when you receive your bill.

Lakeshore Health Connect


Sign up for our new patient portal Lakeshore Health Connect to make a payment or view your health records online for services you received after 2/28/22.

- Schedule and check appointment dates.
- Communicate with your providers.
- Access your test results and after visit summaries.
- Request prescription refills.

Lakeshore Health Connect services are provided by MyChart by Epic. You will be take to MyChart, a secure website, to sign in:

Effective March 1, 2022, Lakeshore Community Health Care will no longer be using Advocate Aurora Health’s LiveWell patient portal. You can login to LiveWell to access your previous records, but you will not be able to see new records, test results or communicate with your provider. However, in your new account you can connect to other patient portals you have records with and view all of your records through LAKESHORE HEALTH CONNECT.

You need to request an activation code to create a new LAKESHORE Health Connect account. The Online Sign Up instructions above include screenshots to help you sign up. If you have questions, please call 920-783-6633.
