We place our patients at the center of their care team - made up of doctors, dentists, mental health professionals, pharmacists and case managers - to get a full understanding of each patient’s unique needs.

A personalized health plan is designed for each patient. Case managers link patients to community resources to help them secure housing, healthy food, employment, insurance, and other needs that may impact your overall health.

Dr. Allison Kos, DO
Chief Medical Officer

Residency training in Family Medicine at St. Vincent’s Hospital – Erie, PA

I was born in the Midwest but lived most of my life near DC and in upstate NY. I went to medical school in Maine at the University of New England and trained in Family Medicine at a community hospital in PA. 

I’ve been with Progressive CHC, in Milwaukee, since 2006 and the Chief Medical Officer for over a decade. I’m passionate about seeing FQHCs thrive, highlighting strong clinical care and quality, and creating impact in communities. I joined LCHC as Chief Medical Officer, providing department guidance for LCHC's medical team.

I’m fortunate to share my life with my husband and young daughter.  We enjoy fresh air activities, music and trying new adventures. One of my favorite quotes: “Start by doing what is necessary, then what is possible, and suddenly you are doing the impossible.”

I grew up in Wisconsin and am a so called “Wisconsin Lifer”. I’m excited to continue practicing here as well with LCHC to provide access to integrated health care to the community.

I would like to work with my patients as a team towards their health goals. I look forward to caring for my patients, their family members, and watching multiple generations grow throughout their lives. My interests include chronic disease prevention and management, geriatrics, women’s health, pediatric/adolescent medicine and mental health.

I come from a huge family, whom I enjoy spending time with.  I also like to spend time outdoors. Favorite quote:

I was born and raised in Sheboygan Falls and have 26 years of nursing experience. I have a history of participating in Mission trips through my church. Since that time, I've looked for a place with this same type of mission. I found it at Lakeshore Community Health Care.

I assist patients and families with their health care goals and connect them to resources - a safe living environment, nutritional food and more -  improving their quality of life.

I walk daily, in the summer I like to hike trails in the Kettles outside of Plymouth. I enjoy picking an area of Wisconsin that my family hasn't been to for a while, and seeing what's new in our state.

Community healthcare is my passion and I can't imagine working in another area! I want my patients to feel that they are cared for and seen as unique individuals, not a number.

I've worked in various roles including home care, long term care, supervision and medical surgical. I was an RN Supervisor at HSHS St. Mary's in Green Bay, an Emergency RN at Holy Family Memorial and worked at HSHS St. Nicholas Hospital in Sheboygan.

My husband and I have four adult children, a foster son and 8 grandchildren. I do Crossfit, participate in Mud Runs and 5Ks. I love to sing and enjoy spending time with my family.
